With 9.16 I am unable to adjust the top tube extension above or below head tube and seat tube extension above top tube. It only has from center of intersection. Please advise.

As long as there are no curved tubes in the front triangle, you can toggle that control from this option
to this option 
. There is an explanation of this at: bikecad.ca/tube_extension. Let me know if that doesn't resolve your issue.
